Our service can provide a wide range of options for people already living independently in the community, or for those who wish to move from supported accommodation. Stepping Stone Trust Community Support Workers provide assistance in developing life skills, obtaining benefit entitlements, developing networks with family and friends and the wider community. Referral to other community and mental health services, and assistance in accessing other support that may be required is an important part of the service.
Those aged between 20-65 years can access our adult team. Those aged 14-19 years can access our youth C.S.W.'s.
You need to have a caseworker within the health services.
We provide this service within your home or at a place which is suitable to both parties. For adults you would normally see your worker once a week with further phone contacts made as necessary. For youth, your worker would visit and ring more often.
We have 18 full time community support workers providing assistance for up to 200 clients. The staff work Monday to Friday with a flexibility of meeting times between 8.30am and 7.00pm.
A needs assessment is required to access these services which is available through Lifelinks, Toiora Arotake or Needs Coordination Centre at Princess Margaret Hospital. There is no cost for using this service. |
